For participants

The Douglas Institute, an affiliate of McGill University is conducting a confidential survey on health issues and well-being in diabetes. The results will help improve existing diabetes health services in Québec.

The objective of this study is to examine health and well-being, use of health services, and some of the factors that can affect physical and emotional health in people with diabetes. We hope that our findings will generate a change in the way that diabetes is currently managed. The goal is to incorporate a focus on treating emotional symptoms as well as the physical ones and ensure the well-being of current and future diabetes patients in the province of Québec.

The study is sponsored by the Canadian Institutes of Health Research. Your participation is essential for the accuracy of the results. All information collected will be used only for the purposes of this study. All participant information remains strictly confidential. This includes any information you provide over the telephone in the survey, as well as the information obtained from RAMQ.

Follow-up

The follow-up interview is very important for the study and for gaining a better understanding of health status in diabetes. Your participation is greatly appreciated. If you agreed to participate in the follow-up portion of the study, you will be contacted between February and March 2013.
